If you have MTHFR, there is a good chance you have heard that folic acid is bad for you. But when you are standing in the supplement aisle staring at a wall of options, it can be genuinely confusing to understand why, especially when folic acid is in almost every multivitamin, every fortified food, and every prenatal vitamin on the shelf.
This post explains the difference clearly, without the overwhelm, so you know exactly what to look for and what to avoid.
First, What Even Is Folate
Folate is the umbrella term for vitamin B9, an essential nutrient your body needs for DNA synthesis, cell division, methylation, neurotransmitter production, and a long list of other critical functions. Your body cannot make it on its own so it has to come from food or supplements.
The key word in all of this is active. Folate from food and folate from supplements are not automatically in a form your body can use directly. Most forms have to go through a conversion process first before your cells can actually put them to work.
This is where MTHFR becomes the problem.
What Folic Acid Actually Is
Folic acid is a synthetic form of vitamin B9 produced in a laboratory. Unlike naturally occurring folate, it requires conversion by a specific enzyme in the body to become active.
That enzyme is the MTHFR enzyme, the one your mutation directly impairs.
Folic acid is the synthetic form of folate that is often in supplements and in fortified foods such as cereals. There is a gene called MTHFR that helps produce an enzyme that converts a portion of folate and folic acid into folinic acid, which is then converted into the active form methylfolate used by your cells.
So the conversion chain looks like this: folic acid goes in, the MTHFR enzyme tries to convert it, and eventually it should become methylfolate, the active form your cells actually use.
The problem is that if your MTHFR enzyme is not working efficiently, that conversion stalls. MTHFR C677T homozygotes have roughly 70% reduced MTHFR enzyme activity, the step that produces active methylfolate.
Which means you swallow the folic acid, your body struggles to convert it, and you are left with unconverted folic acid building up in your system rather than active methylfolate getting to where it needs to go.
What Methylfolate Actually Is
The key distinction is that folic acid is a synthetic form while 5-MTHF is the natural and physiologically active form of folate.
Methylfolate (also called 5-MTHF or L-methylfolate) is already in its active form. It does not need the MTHFR enzyme to convert it. It bypasses the whole conversion process and goes straight to work in your cells regardless of your genetic status.
5-MTHF supplementation bypasses the MTHFR conversion step, achieving folate status independent of genotype.
This is why methylfolate is the supplement form recommended for people with MTHFR. Not because folic acid is inherently evil, but because for people with impaired MTHFR enzyme activity, folic acid cannot reliably get converted into what your body actually needs.
Why This Matters Beyond Just Taking a Supplement
Here is the part most articles leave out: folic acid is not just in supplements. It is in the food supply.
Many countries mandate folic acid fortification in staple foods as a public health strategy to reduce neural tube defects. This means it is in bread, pasta, rice, cereal, flour, and a huge range of packaged foods whether you can see it on the label or not.
If you have MTHFR and your conversion pathway is impaired, you are not just dealing with the folic acid in a multivitamin. You are dealing with a daily background load of unconverted folic acid from fortified foods throughout your entire diet.
This is one reason reducing processed and packaged foods matters for people with MTHFR, not just because of general wellness principles, but because of how much accumulated synthetic folic acid can be coming in through the food supply without you even realizing it.
The Fortified Food Problem
Check the labels on these common items and you will almost certainly see folic acid listed:
Bread and flour. Pasta and rice. Breakfast cereals. Nutritional yeast. Most conventional multivitamins. Most prenatal vitamins. Most B complex supplements. Many energy drinks and fortified beverages.
For people without MTHFR this fortification is a genuine public health benefit, it has significantly reduced neural tube defects in populations where it is mandated. For people with MTHFR it is something worth knowing about and working around where possible.
My Own Experience
I found out about the folic acid and MTHFR connection at the same time I found out I had MTHFR. It came up in the same late night scrolling session that changed everything for me, videos explaining the symptoms I had been living with my whole life and why folic acid was specifically a problem for people like me.
I was not taking any supplements at the time. I had been on what felt like a mountain of prescriptions for years, none of which were actually addressing a root cause, but no targeted supplementation. Earlier in my adult life I had tried a multivitamin that almost certainly contained folic acid, but the pills were so large I could not swallow them so that did not last long anyway.
Once I tested positive for MTHFR and started taking methylfolate with B12, at a child-sized starting dose because you genuinely do need to start low and go slowly, I felt a difference within a couple of weeks. That is not a small thing after thirty years of feeling the way I felt.
Starting low matters. At dosages above 1,000 mcg, methylfolate may stress methylation pathways, leading to feelings of anxiety, irritability, and insomnia. This is what overmethylation feels like and it is why starting at a low dose and increasing slowly is so important. If you have tried methylfolate and felt anxious or wired, that could be why, and the fix is a lower starting dose, not giving up on it entirely.
What to Look For on Labels
Now that you know the difference, here is how to actually use this information when buying supplements or reading food labels:
Avoid:
- Folic acid
- Pteroylglutamic acid (another name for folic acid)
Look for:
- Methylfolate
- L-methylfolate
- 5-MTHF
- 5-methyltetrahydrofolate
- Quatrefolic (a branded form of methylfolate)
In food labels:
- “Enriched” flour, rice, or pasta almost always means folic acid has been added
- “Fortified” cereals and grains typically contain folic acid
